I am torn between including them or not.  Parents might get turned off by a waiver and putting fear in them.  While in the church's perspective, this will be beneficial for us just in case something happens.

Our registration forms include liability waiver information, and permission to use photos in church bulletins, etc. Nowadays, I think people are use to the need for this type of authorization/waiver.
