I'm looking for some examples of youth ministry intern applications. I plan on taking on two interns this summer but need some guidance.

This is just a simple job description I use for mine. The application should be a resume if you ask me. Then formal interviews from there.
Job Title: Student Ministries Intern/Associate
Reports To: Director of Student Ministries
Status: Temporary Part-Time

SUMMARY
This is a 20-30 hour per week position that is completely responsible for the development of the student ministry. Under the leadership of the Director of Student Ministries, the intern would be responsible for planning, facilitating and evaluating the following ministry programs.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
• Wednesday night
-outreach ministry
-small group ministry
• Sunday School class for junior high students that teaches Biblical principles which apply to their lives.
• Provide opportunities for one-on-one meetings with the youth for the purpose of getting to know them on a personal level.
• Participate in a vigorous visitation program for all active, inactive, and prospective members of junior high youth ministries.
• Develop an effective follow-up program to maintain a high level of interest.
• Encourage, communicate with, and provide resources to the parents of our teenagers.
• Create and oversee various outreach events such as lock-ins, game nights, concerts, camp, etc.
• Develop discipleship relationships with select students.
QUALIFICATIONS - To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required. EDUCATION and/or EXPERIENCE
Completed High School or High School equivalent. Have a calling to the ministry. Have some sort of youth ministry experience in a church setting.
OTHER SKILLS AND ABILITIES
The successful candidate must be a Christian and fully support the philosophy of First Baptist Church and its leadership team. This individual's commitment to Christ will be evident in his/her personal life, family relationships, and ministry. His/her daily walk with God will be an example to the students with which he/she interacts.
Must understand the maturity levels and characteristics of teens and the challenges they face today.
The ability to work efficiently and effectively in team situations is critical, as well as the ability to work independently.
Must possess solid time management skills and organizational abilities. The ability to coordinate several activities at once and to quickly analyze and resolve specific problems is important.
The ideal candidate should be creative, have a passion and heartfelt concern for teens, be high energy, hard working, and possess the ability to motivate others.
